Zusammenfassung
Hintergrund: Faltbare intraokulare Silikonlinsen haben im Zusammenhang mit der Kleinschnittchirurgie und Phakoemulsifikation der Katarakt an Beliebtheit gewonnen.
Patienten: Wir beobachteten drei Patienten mit visusbeeinträchtigenden Auflagerungen von Si-likonöltröpfchen auf der Silikonlinse nach Silikonölablassung wegen vitreoretinaler Chirurgie mit Silikonölendotamponade. Operativ war es nicht möglich, diese Silikonöltröpfchen von der Silikonlinse zu beseitigen. Wir haben in diesen drei Fällen einen Austausch der Silikonlinse gegen eine PMMA-Linse vorgenommen.
Ergebnisse: Der Eingriff führte zu einem Wiederanstieg der Sehschärfe. Die Mechanismen, welche zur Adhäsion der Silikonöltröpfchen an der Silikonlinse führen, sind letztlich nicht geklärt.
Schlußfolgerungen: Mit zunehmender Verbreitung der Silikonlinsen wird in Zukunft häufiger mit diesem Problem zu rechnen sein. Daher sollte die Indikation zur Implantation von Silikonlinsen in Augen mit erhöhtem vitreoretinalen Risiko kritisch gestellt werden.
Summary
Background: Foldable silicone intraocular implants are becoming increasingly popular in conjunction with small incision phacoemulsification.
Patients: We observed three patients with silicone oil droplets adherent to the posterior surface of the silicone implant, following silicone oil removal after preceding vitreoretinal surgery with the installation of silicone oil. These droplets could not be dislodged intraoperatively either with focal aspiration or irrigation. Since they interfered with the patients’ subjective visual acuity, we exchanged the silicone intraocular implant for a PMMA one-piece lens in all three patients.
Results. This procedure resulted in an increase of the visual acuity. The mechanism of adherence between the two silicone polymers is not known.
Conclusions. With increased use of implantable silicone intraocular lens and silicone oil, this complication may be encountered more frequently. Therefore, implantation of silicone lens in vitreoretinal high risk eyes should avoided.
