Zusammenfassung
Einen Monat nach Phakoemulsifikation und Implantation einer Silikonlinse entwickelte sich bei einer 79jährigen Patientin eine ausgeprägte Hypotonie mit einem IOD von 1–2 mm Hg. Echographisch war der Ziliarkörper zirkuläre abgehoben. Gleichzeitig bestand eine starke Vorderkapselschrumpfung mit radiärer Kapselfältelung. Nach Nd-YAG-Laserkapsulotomie des anterioren Fibroserings stieg der IOD schlagartig auf normale Werte. Es handelte sich offensichtlich um eine durch Zug verursachte Ziliarkörperabhebung. Das seltene Auftreten dieses Phänomens bei starken Kapselschrumpfungen könnte durch die Elastizität der Zonulafasern erklärt sein. Normalerweise geben sie nach; im vorliegenden Fall lag aber eine Pseudoexfoliation vor, so daß die Fasern starr und unelastisch waren und die Zugkräfte vollständig auf den Ziliarkörper weiterleiteten.
Summary
In a 79-year-old patient we observed the development of a severe hypotony (IOP 1–2 mm Hg) one month after phacoemulsification and implantation of a silicone lens. The ciliary body showed a circular detachment by means of echography. Furthermore, the anterior capsule demonstrated advanced shrinkage with radiar folds. After Nd-YAG laser capsulotomy of the anterior ring of fibrosis, the IOP normalized at once. Obviously, the cause of hypotony was a tractional detachment of the ciliary body. The low frequency of this finding in cases of severe shrinkage of the lens capsule may be explained by the elasticity of the zonula fibers. Normally they are stretched. The presented patient had Pseudoexfoliation (with normal IOP), and the rigid and nonelastic zonula fibers transmitted the tractional forces completely to the ciliary body.
