Zusammenfassung
An den Universitäts-Augenkliniken in Kiel, Gießen und Köln wurden im Rahmen einer prospektiven multizentrischen Studie insgesamt 138 multifokale Intraokularlinsen vom Typ „Array“ implantiert. Folgende Parameter wurden beurteilt: Fernvisus, Nahvisus, Kontrastempfindlichkeit mittels Regan-Kontrasttafeln (96, 50, 25 und 11%), Blendungsempfindlichkeit mittels Brightness acuity tester (BAT) für die Ferne. 13% der Augen erreichten ihren besten Fernvisus ohne Korrektur. Der unkorrigierte Fernvisus lag im Durchschnitt bei 0,64, der korrigierte bei 0,97. 36% der Patienten konnten kleinste Schrift (Nieden 1–3) ohne zusätzliche Addition lesen. Für den besten Nahvisus war im Durchschnitt eine Nahaddition von + 2,2 dpt erforderlich. Bei der Untersuchung der Kontrastempfindlichkeit zeigte sich für die 50%-Regan-Kontrasttafel eine Visusreduktion um durchschnittlich 13% gegenüber der 96%-Tafel. Die Tests zur Blendungsempfindlichkeit ergaben für die 96%-Kontrasttafel bei mittlerer und hoher Blendungsintensität eine Visusreduktion um 7 bzw. 19% gegenüber dem Visus ohne Blendung. Bei der 11%-Kontrasttafel und hoher Blendungsintensität lag die Visusreduktion bei 90%. Der mit der AMO-Array-IOL erreichte sowohl korrigierte als auch unkorrigierte Fernvisus entspricht weitgehend der mit monofokalen und anderen bi- bzw. multifokalen IOL erreichten Sehschärfe. Da bei der Array-IOL die Dominanz auf der Fernsehschärfe liegt, ist für das Lesen kleinster Schrift eine Nahaddition erforderlich.
Summary
A prospective study was done to assess the performance of 138 refractive multifocal intraocular lenses (IOL) of the Array type. The following parameters were evaluated: distance visual acuity, near visual acuity (Nieden), contrast sensitivity (Regan charts 96%, 50%, 25%, n%), glare (brightness acuity tester, BAT) for distance visual acuity. In 13% of the patients, the best distance visual acuity was achieved without any correction, while 36% did not need any correction for their best near visual acuity. Overall, an average correction of + 2,2 D was necessary for best near visual acuity. The average uncorrected distance visual acuity was 0,64 and the average best corrected distance visual acuity was 0,97. In contrast sensitivity testing with the 50% Regan chart, visual acuity decreased by an average of 13% compared with the 96% chart. When glare was tested with BAT at the “high” position the average visual acuity compared to that without BAT decreased by 19% for the 96% chart and 90% for the 11% chart. The Array type IOL allowed good distance and near visual acuity. All our patients were pleased with their intermediate visual acuity without glasses.
