Zusammenfassung
Die Phakoemulsifikation mit Intraokularlinsenimplantation durch eine Skierai- oder Hornhauttunnelinzision induziert einen minimalen Astigmatismus und garantiert eine frühe postoperative visuelle Rehabilitation. Hornhauttunnelinzisionen einer Breite von größer als 3,5 mm, z. B. für die Implantation von PMMA-Intraokularlinsen, bedingen möglicherweise eine geringere Wunddichtigkeit mit höherem Endophthalmitisrisiko.
Eine Alternative zum Wundverschluß mit Naht in der Kataraktchirurgie stellt die Abdichtung von Hornhaut- und Skleratunnelinzisionen mit Fibrin nach Phakoemulsifikation und Implantation verschiedener PMMA-Intraokularlinsen dar.
Der Fibrinkleber besteht aus zwei Komponenten: 1. humanes Fibrinogen, Faktor XIII mit Aprotinin und 2. Thrombin mit Kalziumchlorid. Beide Komponenten werden intraoperativ in einer Mischspritze vereinigt und führen zur Einleitung der Endreaktionen der Koagulationskaskade, die in der Bildung von Fibrin endet. Der Kleber härtet nach einigen Sekunden. Bereits die Applikation einer sehr geringen Menge reicht zur vollständigen Abdichtung aus. Der nichtverwendete Rest bleibt unverbraucht in dem Spritzensystem bis zur nächsten Applikation zurück. Der Fibrinkleber erzielte einen raschen und wasserdichten Wundverschluß, ohne daß eine Naht erforderlich wurde. Es fand sich auch nach Monaten ein nur geringer chirurgisch induzierter Astigmatismus.
Summary
Longer incisions for IOL implantation are associated with less self-sealing properties and a possibly higher risk of infection. Our intention was to reinforce the scleral and clear-corneal tunnel incision after phacoemulsification and IOL implantation with fibrin adhesive. Upon simultaneous mixing of fibrinogen complex and thrombin using a double syringe, the final stages of the coagulation cascade are mimicked after application of fibrin adhesive, resulting in fibrin clot formation. The sealant consolidated and adhered to the site of corneal application. Fibrin adhesive is efficacious in rapid and watertight wound closure and provides good support for wound healing. Surgically induced astigmatism was minimal. This technique of corneal wound closure is a simple and effective alternative to single-suture wound closure after small incision cataract surgery.
