Zusammenfassung
Bei einem Patienten wurde als Ursache einer zunehmenden Sehver-schlechterung eine sektorformige hintere Schalentrübung festgestellt, die durch einen die Linse eindellenden Ziliarkörpertumor verursacht wurde. Um eine iatrogene Tumorzellpropagation zu vermeiden, wurde die Resektion des Tumors zuerst durchgeführt. Bei der Blockresektion mußte die Zonula in einer Ausdehnung von einem Quadranten durchtrennt werden. Als wei-teres Erschwernis der 10 Wochen später durchgeführten Kataraktoperation trat der Umstand hinzu, daß die Katarakt mittlerweile intumeszent geworden war. Trotz des Einsatzes eines Hochfrequenzzystotoms entstanden zwei radiäre Einrisse im Verlauf der vorderen Kapsuloto-mie. Nach Einsetzen eines Spannringes wurde die Katarakt emulsifiziert. Rindenaspiration und Kapselpolitur waren problemlos durchführbar. Die durch den sklerokornealen Ventilschnitt in den Kapselsack eingebrachte Ein-Stück-PMMA-Linse zentrierte spontan und dauerhaft.
Schlußfolgerung: Der Kapselspannring erlaubt die sichere Durchführung der Kataraktoperation und die zentrische Fixation einer Kapselsacklinse selbst dann, wenn die Kapsulotomieöffnung radiäre Einrisse aufweist. Er ist besonders hilfreich bei Kataraktoperationen nach Blockresektion von Ziliarkörpertumoren.
Summary
A patient presented with a ciliary body tumor clinically suspicious of melanoma. The tumor was located in the inferonasal quadrant and had invaded the iris base and chamber angle; due to lens contact, a vision-impairing posterior subcapsular cataract had formed. In order to avoid the risk of systemic cell dissemination, bloc resection of the tumor was done first requiring transsection of the zonular fibers over one quadrant. In the following months, cataract progressed to intumescence. Despite the use of a high-frequency cystotome, anterior capsulotomy resulted in two radial tears. Before emulsification and aspiration of the cataract, a capsule tension ring was inserted in order to prevent bulging of the posterior capsule as well as extension of the dialysis. The one-piece C-loop PMMA lens inserted through the sclerocorneal lip incision centered spontaneously and permanently. Thus, the capsule tension ring allows for safe cataract surgery and stable lens centration with zonular defects, even if the anterior capsulotomy is radially torn. It is specifically helpful in cataract surgery following cyclectomy for ciliary body tumors.
