Zusammenfassung
Um die Stabilität größerer Frown-Inzisionen zur Implantation von PMMA-Linsen mit großer Optik zu verbessern, wird eine Modifikation vorgeschlagen: dabei wird die Sklera nicht in Form eines umgekehrten U, sondern eines W vorinzidiert. Dann folgt die Tunnelpräparation in der üblichen Weise. Diese Inzision hat alle Eigenschaften einer konventionellen Frown-Inzision, aber zusätzlich noch einen zentralen skieralen Lappen. Dies erlaubt einen Einzelnahtverschluß weit weg vom Limbus. Über 400 Augen wurden so operiert. Die Vorteile der W-Inzision sind: weitere Verbesserung des Astigmatismus, verbesserte selbstschließende Funktion und geringere postoperative Leckage, Anspruchslosigkeit des W-Verschlusses, hohe mechanische Stabilität und leichte Kombinierbarkeit mit einer Trabekulektomie.
Bei der kombinierten Katarakt-Glaukom-Operation wird lediglich zusätzlich unter dem Läppchen eine Trabekulektomie und periphere Iridektomie angelegt.
Summary
Larger frown-type incisions used for implantation of rigid posterior chamber lenses still bear certain disadvantages as far as stability and astigmatism are concerned. A suture in the center of the incision is thereby not able to optimally solve these problems. Therefore we suggest a modification. A scleral groove is made away from the limbus which resembles an inverted W and the anterior chamber is entered by a conventional tunnel technique. This tunnel bears all the characteristics of a frown incision but has an additional trangular central scleral flap. On closure of the incision, a single stitch can be placed through this flap away from the limbus.
The advantages are as follows: further reduction and control of astigmatism, improved selfsealing characteristics, reduced postoperative leakage, no special requirement for suture material, high mechanical stability, easily extendible for trabeculectomy. For combined cataractglaucoma intervention, the trabeculectomy and peripheral iridectomy can easily be done underneath the flap.
