Zusammenfassung
Hintergrund: In einer früheren Studie fanden wir, daß der Druckanstieg nach extrakapsulärer Kataraktextraktion wesentlich ausgeprägter ist als nach Phakoemulsifikation. Wegen der geringen Zahl extrakapsulär operierter Augen, die außerdem verschiedene drucksenkende Medikamente erhalten hatten, wurde die Studie angezweifelt. In der vorliegenden, prospektiven Studie untersuchten wir daher den postoperativen Druckanstieg bei verschiedenen Operationstechniken anhand einer ausgeglichen Stichprobe.
Patienten und Methoden: Bei jeweils 30 Patienten führten wir entweder eine Phakoemulsifikation mit korneoskleralem Tunnel (Phako/Tunnel), eine Phakoemulsifikation mit Korneo-skleralschnitt und Naht (Phako/Naht) oder eine extrakapsuläre Extraktion mit Korneoskleral-schnitt und Naht (ECCE) durch. Der Augendruck wurde einen Tag vor sowie 5–7 und 22–24 h nach der Operation mit einem Goldmann-Tonometer in der o°- und 90°-Achse gemessen.
Ergebnisse: In allen Gruppen stieg der mittlere Augendruck signifikant (p immer < 0,05) auf ein Maximum 5–7 h nach der Operation an. Auch nach 22–24 h war die Druckerhöhung noch signifikant (p immer < 0,05). Der mittlere Druckanstieg nach 5–7 h war in der ECCE-Gruppe (20,2 ± 7,9 mm Hg) signifikant (p < 0,005) höher als in der Phako/Naht-Gruppe (11,2 ± 8,3 mm Hg) und der Phako/Tunnel-Gruppe (4,7 ± 8,7 mm Hg) sowie in der Phako/Naht-Gruppe signifikant (p < 0,05) höher als in der Phako/Tunnel-Gruppe. Ein Druckanstieg ≧ 40 mm Hg trat nach 5–7 h signifikant häufiger (p < 0,01) in der ECCE-Gruppe (9 von 30) als in den beiden Phako-Gruppen (jeweils 1 von 30) auf.
Schlußfolgerungen: Die Ergebnisse bestätigen den ausgeprägten Einfluß der Operationstechnik auf den Druckanstieg nach Kataraktextraktion.
Summary
Background: In a previous study, we found that the rise in intraocular pressure (IOP) after cataract extraction strongly depends on the surgical technique. However, due to various medications and the small number of extracapsular operations performed, this result has been questioned. In the present prospective study we therefore compared the rise in IOP for different surgical techniques in a balanced sample.
Patients and Methods. Patients underwent either phacoemulsification and corneoscleral sutureless tunnel (phaco/tunnel), phacoemulsification, corneoscleral incision and suture (phaco/suture), or an extracapsular technique including corneoscleral incision, nucleus expression and suture (ECCE). Each group contained 30 patients. IOP was measured the day before as well as 5–7 h and 22–24 h after surgery.
Results: In all groups, the mean IOP significantly increased to a maximum 5–7 h after surgery. The pressure rise was significantly (p < 0,005) higher in the ECCE group (20.2 ± 7.9 mm Hg) than in the phaco/suture (11.2 ± 8.3 mm Hg) and the phaco/tunnel (4.7 ± 8.7 mm Hg) group and significantly (p < 0,05) higher in the phaco/suture than in the phaco/tunnel group. 22–24 h after surgery, the IOP was still significantly elevated (p < 0,05). IOP peaks ≥ 40 mm Hg at the 5–7 h measurement were found significantly (p < 0,01) more often in the ECCE group (9/30) than in either phaco group (each 1/30).
Conclusions: Our data confirm that the rise in IOP after cataract extraction strongly depends on the surgical technique.
