Zusammenfassung
Einleitung: Als postoperative Komplikation einer Kataraktoperation mit Hinterkammerlinsenimplantation (HKL-Implantation) wurde in den letzten Jahren ein ziliolentikulärer Blockmechanismus beschrieben, wie er nach fistulierenden Operationen bei Glaukompatienten bekannt ist. Kasuistiken: Die Entwicklung eines iridoziliokapsulovitrealen Blocks (IZKVB) wurde bei drei Patienten nach Kataraktoperation mit HKL-Implantation beobachtet. Bei zwei Patienten war 18 Jahre zuvor eine fistulierende Operation aufgrund eines primären Offenwinkelglaukoms mit im Zugang eingeengtem Kammerwinkel bei relativem Mikrophthalmus durchgeführt worden. Bei dem dritten Patienten waren mehrmalige Iridotomien aufgrund eines Sekundärglaukoms bei rezidivierenden Iridozyklitiden vorgenommen worden. Die Kataraktoperation verlief jeweils komplikationslos. Zwei, elf und zwölf Monate postoperativ kam es zur Vorderkammerabflachung und Tensionsanstieg. Pars-plana-Vitrektomie und hintere Kapsulotomie führten zur Vorderkammertiefung und Tensionsregulierung.
Schlußfolgerung: Bei sehr kleinem vorderen Augensegment und/oder entzündlichen Veränderungen sowie durch Voroperationen können sich Adhäsionen im Bereich der iridoziliokapsulovitrealen Gewebestrukturen ausbilden. Es kommt zur Umkehrung der Kammerwasserzirkulation nach glaskörperwärts mit Zunahme des Glaskörpervolumens, Abflachung der Vorderkammer und Tensionsanstieg. Unter Berücksichtigung der Pathogenese stellt die kombinierte Pars-plana-Vitrektomie und hintere Kapsulotomie ein wirksames operatives Verfahren dar.
Summary
Introduction: A recently occurring postoperative complication in cataract surgery with posterior chamber lens implantation is ciliary lenticular block, which is found after fistulating operations in glaucoma patients.
Case reports: After cataract surgery, three patients developed an irido-cilio-capsular vitreal block. Two patients had a history of bilateral narrow-angle glaucoma with relative microphthalmia and had undergone glaucoma surgery of the filtering type. In one patient with a history of recurrent iritis, iridotomies were performed due to secondary glaucoma. The cataract operation and the postoperative course were without complication. Some 2, 11, and 12 months after the operation, shallowing of the anterior chamber and elevation of the IOP occurred. After pars plana vitrectomy and capsulectomy, the anterior chamber deepened and the IOP normalized.
Conclusion: In small anterior eye segments and/or a history of inflammation or previous operations, adhesions may occur in irido-cilio-capsularvitreal structures and act as a barrier to the aqueous humor flow. The latter enters the vitreous with a subsequent increase of the vitreous volume. The result is a flattening of the anterior chamber and IOP elevation. In view of the pathogenesis, a pars plana vitrectomy and posterior capsulectomy seem to be reliable surgical methods. hinterfläche wird so blockiert. Auch die alleinige Vitrektomie halten wir nicht für ausreichend, da sich weiterhin Kammerwasser im Glaskörperraum sammelt. Durch eine Pars-plana-Vitrektomie in Kombination mit einer hinteren Kapsulotomie können normale Kammerwasserflußverhältnisse wiederhergestellt werden.
