Definition, Diagnostik und Therapie schizophrener Minussymptomatik

Zusammenfassung

Die Diskussion um die Existenz distinkter Subtypen innerhalb der schizophrenen Erkrankung ist sehr alt und hat mit angloamerikanischer „ Positiv -negativ -Dichotomie“ -Reaktualisierung neue Intensität erlangt (Wing u. Brown 1970; Strauß et al. 1974). Durch die von Crow aufgenommene Dichotomisierung der Schizophrenie in einen Typ I und Typ II kam es zu einer weiteren Akzentuierung der wissenschaftlichen Diskussion (Crow 1980), wobei eine verbindliche Einteilung bisher nicht vorliegt. Zwischenzeitlich bestehen Bedenken, ob die Positiv-Negativ-Klassifizierung den erkrankten Patienten wie auch insbesondere der Schizophrenieforschung gerecht wird (Peralta et al. 1992). Insbesondere das Verknüpfen dieses Subtypenmodells mit ätiopathogenetischen, prognostischen und therapeutischen Hypothesen (Strauß et al. 1974; Angrist et al. 1980; Crow 1985) ließ bei Patienten mit prädominanter Minussymptomatik und der damit verbundenen Etikettierung als „therapierefraktär“ therapeutisch-konzeptionelle Perspektiven vermissen. Katamnese-studien erbrachten zwischenzeitlich Hinweise, daß Plus- und Minussymptomatik zur selben Zeit bei dem erkrankten Patienten vorkommen, demnach nicht gesonderten Krankheitsprozessen zugehörig sind oder sich nicht nur in unterschiedlichen Verlaufsabschnitten einer schizophrenen Psychose ereignen. Nach einer Fokussierung auf diagnostische und ätiopathogenetische Aspekte der schizophrenen Minussymptomatik traten nun auch pharmakotherapeutische Überlegungen zur schizophrenen Minussymptomatik in den Vordergrund wissenschaftlichen Interesses. Allgemeingültige Therapieprinzipien oder -standards der Minussymptomatik bei schizophren Erkrankten bestehen derzeit noch nicht, da

  1. 1)

    schizophrene Minussymptome multifaktoriell bedingt sind,

  2. 2)

    geeignete Tiermodelle für die Negativsymptomatik nicht zur Verfügung stehen und

  3. 3)

    die meisten der bislang durchgeführten Therapiestudien durch erhebliche methodische Probleme gekennzeichnet sind: So war das Studiendesign der bislang publizierten Untersuchungen meist so konzipiert, daß primär das Ansprechen von Plussymptomatik auf Neuroleptika und erst sekundär das Ansprechen der Negativsymptomatik geprüft wurde. Ein weiteres methodisches Problem der vorliegenden Studien, das die Erstellung therapeutischer Richtlinien erheblich erschwert, stellen der geringe Stichprobenumfang und der häufig kurze, nur wenige Wochen betragende Untersuchungszeitraum dar.
