Zusammenfassung
Die photodynamische Therapie (PDT) ist ein neues lokales, endoskopisch kontrolliertes Behandlungsverfahren für Karzinome. Es beruht auf der intravenösen Applikation eines sich vornehmlich in Tumorgewebe anreichernden Photosensibilisators und der nachfolgenden Aktivierung dieser Substanz mit Licht einer spezifischen Wellenlänge. Die bisher am häufigsten angewandten Photosensibilisatoren sind das Hämatoporphyrin-Derivat (HpD) und Dihämatoporphyrin-Ether/Ester (DHE). Die Substanzgruppe der Hämatoporphyrine erfüllt jedoch bei weitem nicht die Anforderungen an einen idealen Photosensibilisator: Es besteht nur eine relative Selektivität aufgrund des geringen Konzentrationsgradienten zwischen Tumor- und Normalgewebe (1). Ein sehr langsames Abklingen der Hämatoporphyrine aus der Haut nach systemischer Applikation bewirkt eine gesteigerte Lichtempfindlichkeit der Haut, die einzige wirklich nennenswerte Nebenwirkung der PDT. Wegen der geringen Absorption der Hämatoporphyrine von Laserlicht längerer Wellenlänge ( >600 nm) limitiert sich die Penetrationstiefe der PDT derzeit auf ca. einen Zentimeter (2). Neue Photosensibilisatoren der zweiten und dritten Generation mit verbesserter Tumorselektivität und erhöhter Gewebepenetration scheinen dem Anspruch an einen idealen Photosensibilisator eher gerecht zu werden. Ziel dieser Studie war es deshalb, unter standartisierten Versuchsbedingungen erstmalig die Effektivität von meso- Tetra- (Hydroxyphenyl) — Chlorin (mTHPC) und Delta- Aminolävulinsäure (δ- ALA) mit DHE in einem in-vivo-Tumormodell zu vergleichen.
