Coping als Wirkfaktor: eine Interventionsstrategie bei somatischen Krankheiten

Zusammenfassung

Mit gewisser Berechtigung wird heute angenommen, daß in vielen Wissenschaftsbereichen ein eigentlicher Paradigma Wechsel eingetreten ist. Innerhalb der Medizin wird der traditionelle (naturwissenschaftliche) Reduktionismus immer mehr durch ganzheitliche Modelle abgelöst (vgl. etwa G. L. Engels Konzept der bio-psycho-sozialen Medizin). Diese Entwicklung hat dazu beigetragen, psychosoziale Faktoren in Ätiologie und Pathogenese vieler Krankheiten neu zu gewichten. Die Copingforschung hat ferner ermöglicht, die Krankheitsverarbeitung besser zu verstehen. Coping wird dabei meist als eine Reaktion auf vielfaltige krankheitsbedingte Belastungen aufgefaßt. Nur ausnahmsweise wird eine Modifikation von Coping aktiv angestrebt, um so das Krankheitsverhalten günstig zu beeinflussen. Diese Möglichkeit verdient es aber, genauer geprüft zu sein, da die traditionellen Psychotherapien nur sehr beschränkt zur Krankheitsverarbeitung beizutragen vermögen; es sei denn, die psychischen Folgen seien so ausgeprägt, daß die gleichen Indikationskriterien gelten würden, wie bei psychischen Störungen allgemein.
