Zusammenfassung
Es wird häufig behauptet, daß ein Gynäkologe, der für die Korrektur urogenitaler Fisteln Erfahrungen sammeln möchte, in ein Entwicklungsland reisen sollte. Möglicherweise stimmt diese Aussage im Hinblick auf geburtshilfliche Fisteln, die in den Vereinigten Staaten nahezu verschwunden sind; demgegenüber müssen aber Fisteln, die durch Operationen und Bestrahlungen verursacht wurden, als ein Übel einer wohlhabenden Gesellschaft betrachtet werden, einer Gesellschaft, die sich eine große Anzahl elektiver Operationen und adäquate Behandlung maligner Genitalerkrankungen leisten kann. Operation oder Bestrahlung oder beides zusammen sind heute in den USA für 95% der Fisteln verantwortlich.
