Zusammenfassung
Die Aspirationspneunomie steht an vorderer Stelle anaesthesie-beclingter schwerer und tödlicher Komplikationen. 66–90% dieser Pneumonien sind bedingt durch das Eindringen von saurem Magensaft in das Bronchialsystem. Entscheidendes Kriterium für die Pathogenese sind ein pH unter 2, 5 und ein aspiriertes Volumen von mehr als 20 ml. In der Vergangenheit wurden eine Anzahl von Vorschlägen zur Vermeidung einer Aspiration bzw. einer Aspirationspneumonie gemacht. Diese Methoden sind jedoch entweder unsicher oder nur bei solchen Patienten mit vollem Magen bzw. Ileus indiziert. Die Applikation von Antazida hat sich sogar als potentiell gefährlich erwiesen. Fast 50% der säurebedingten Aspirationspneumonien wurden bei elektiven Eingriff fen beobachtet. Die Applikation von H2-Rezeptor-Antagonisten wie Cimetidin hat sich in mehreren Studien als effektiv zur Anhebung des Magensaft-pH’s erwiesen. Nachdem sich in einer eigenen Pilotstudie auch die intramuskuläre Cimetidinapplikation als wirksam sowohl hinsichtlich der pH-Anhebung über 2, 5 als auch zur Volumenreduzierung des Magensaftes erwiesen hatte, haben wir in einer offenen Studie bei über 200 elektiv chirurgischen Patienten sowohl die Wirksamkeit als auch die Praktikabilität und Verträglichkeit dieser Prämedikationsart untersucht. In über 98% fand sich ein pH über 2, 5 und in über 90% ein Magensaftvolumen unter 20 ml. Sowohl Verträglichkeit als auch Praktikabilität waren ausgezeichnet. Auch bei geburtshiflichen Narkosen hat sich die i.m. Cimetidinapplikation bewährt. Die routinemäßige Prämedikation mit 400 mg Cimetidin per os zur Nacht sowie dieselbe Dosis i.m. mindestens 2 h vor Narkosebeginn könnte somit eine wirksame Möglichkeit darstellen, die Häufigkeit schwerer und tödlicher Aspirationspneumonien im Rahmen der Allgemeinnarkose zu senken. Zum Schluß werden die therapeutischen Möglichkeiten nach stattgefundener Aspiration diskutiert.
