Zusammenfassung
Im Gegensatz zu peptischen gastroduodenalen Geschwüren sind Streß-Ulcera charakterisiert als akute, meist oberflächliche Schleimhautdefekte ohne bindegewebige Umgebungsreaktionen mit bevorzugtem Sitz im Fundus und Corpus des Magens. Ursache sind schwere physische Belastungen des Organismus infolge schwerer akuter Krankheiten (Polytrauma, große Operationen, großflächige Verbrennungen, Organinsuffizienzen, Sepsis, Schädelhirntraumen). Die pathogenetischen Faktoren sind im einzelnen nicht bekannt. Sehr wahrscheinlich wird die primäre fokale Ischämie und gestörte Zytoprotektion von einer peptischen Andauung der Schleimhaut gefolgt. Unter den verschiedenen möglichen pathogenetischen Faktoren werden neben einer gestörten Durchblutung und Zytoprotektion auch eine Änderung der Säuresekretion, Schleimhautbarriere, Schleimsekretion und duodenogastraler Reflux von Gallensalzen diskutiert. Die Rolle des Histamins ist ebenfalls nicht endgültig geklärt. Die Verhütung und Behandlung richtet sich in erster Linie nach einer optimierten Therapie der Grundkrankheit. In zweiter Linie zielen die empfohlenen Maßnahmen auf eine Reduktion der aggressiven luminalen Faktoren wie Säure, Pepsin und Gallensalze. Aufgrund zahlreicher kontrollierter Studien ist die Kombination von Cimetidin und Antazida der jeweiligen M07 notherapie vorzuziehen. Kommt es unter dieser prophylaktischen Maßnahme zur Blutung, so sollten vor der risikoreichen Operation konservative Maßnahmen (Somatostatin, Sekretin, endoskopische Koagulation) ausgeschöpft werden.
