Zusammenfassung
Ziel der Behandlung bei einer Unterschenkelamputation ist für Chirurgen und Patienten die Rehabilitation des Amputierten bis zu jenem Grad an Unabhängigkeit, den er vor der Operation hatte (Cohen et al. 1974; Molen 1973; McCollough et al. 1971; Robinson 1976). Daß dieses Ziel schwierig zu erreichen ist, ist jedem Arzt, der sich mit Amputationen beschäftigt, klar, dennoch muß man das Ziel während allen Phasen der Behandlung im Auge behalten, also bei der anfänglichen Beurteilung, beim Eingriff, bei der unmittelbar postoperativen Phase und bei der Prothesenversorgung. Behält man dieses Ziel klar im Auge, können viele schwierige Entscheidungen, was die Amputationshöhe betrifft, z. B. die chirurgische Technik, die Intensität des Trainings usw., schon vor dem Eingriff getroffen werden, und die psychologischen Fallen des zu großen Optimismus des Chirurgen und der zu großen Abhängigkeit des Patienten können leichter umgangen werden.
