Zusammenfassung
Wir definieren die Pest beim Menschen als eine durch einen bakteriellen Erreger (Pasteurella pestis) hervorgerufene Anthropozoonose. Ihre tierischen Reservoire stellen freilebende Nagetiere und Hasenartige dar; Flöhe fungieren als Zubringer. Zwei klinische Haupterscheinungsformen sind zu unterscheiden: Beulen-und Lungenpest. Die erste kann in die zweite übergehen, Lungenpest kann aber auch von Mensch zu Mensch durch Tröpfcheninfektion direkt und primär erworben werden. Der Ausbreitungsmodus hängt von der klinischen Erscheinungsform ab und umgekehrt. Die Mortalität der Lungenpest bei unbehandelten und nicht schutzgeimpften Patienten liegt etwa bei 90%. Bei der Ausbreitung der Infektion sind Milieufaktoren verschiedener Art von Fall zu Fall in unterschiedlichem Ausmaß mit im Spiele.
