Zwerchfellpathologie im Röntgenbild

Zusammenfassung

Echte primäre Zwerchfelltumoren sind außerordentlich selten; Seit Grancher 1868 den ersten Fall beschrieb, sind im ganzen nur 57 pathologisch-anatomisch gesicherte Primärtumoren mitgeteilt worden. Die jüngste Übersicht über alle Fälle des Weltschrifttums (Drewes und Willmann) ist mit den von Aufses und Mitarbeiter, Cruickshank, Voluter, Bishop und Mitarbeiter, Clough und Mitarbeiter beschriebenen Geschwülsten zu den Tabellen 1 und 2 erweitert. Mit Ausnahme der vier besonders gekennzeichneten Tumoren, die von embryonal versprengtem Fremdgewebe ausgingen (Gale und Mitarbeiter; Keirns; Griessmann), handelt es sich bei all diesen histologisch bestimmten Tumoren um Geschwulstbildungen aus diaphragmalen Gewebsanteilen selbst. Unsere Tabellen zeigen, daß die bösartigen und die gutartigen Geschwülste — einschließlich der cystisch umgewandelten, vom undifferenzierten Mesothel ausgehenden Tumoren1 — etwa gleich häufig sind. Die Geschlechtsverteilung und die Seitenlokalisation sind gleich. Zur Altersverteilung haben Drewes und Mitarbeiter errechnet, daß die gutartigen Geschwülste das Alter zwischen 30 und 50 Jahren, die bösartigen das zwischen 40 und 60 Jahren bevorzugen; in der letzten Gruppe liegen mit je einem Tumorkranken von 7 und von 80 Jahren auch die Extremfälle.
