Zusammenfassung
Neben dem physiologischen Altersabbau der Endothelzelldichte der Hornhaut führen insbesondere operative Traumen zu einem Verlust an Endothelzellen. Mit der Kontaktspiegelmikroskopie läßt sich in vivo das Verhalten des Hornhautendothels nach Kataraktextraktionen beurteilen. Im Vergleich zur einfachen Kataraktextraktion fällt nach der Implantation von Kunstlinsen ein wesentlich erhöhter Verlust an Endothelzellen auf. Da der Endothelzellverlust unmittelbar postoperativ festzustellen ist, dürfte er nahezu ausschließlich durch das Operationstrauma bedingt sein. Obwohl die kritische Endothelzelldichte beim Menschen noch nicht sicher bekannt ist, sollte bei einer Vorschädigung der Hornhaut auf eine Kunstlinsenimplantation verzichtet werden. Der Endothelzellverlust kann als Indikator für das Ausmaß des Operationstraums gewertet werden und die Implantation von Kunstlinsen anhand dieser Meßdaten in ihrer Technik verbessert werden.
Summary
Besides the decrease of endothelial cell density with aging, surgery of the anterior segment produces a loss of endothelial cells. The clinical specular microscopy allows a follow-up of the corneal endothelium after cataract extraction in vivo. Comparing the normal cataract extraction and those with intraocular implants, the eyes with implants have a significant higher loss of endothelial cells. Recognising this immediately after surgery, we feel the endothelial loss is directly caused by surgical trauma.
