Zusammenfassung
Der herkömmliche Monoculus hält entgegen der allgemeinen Meinung die Lider nicht geschlossen. Die Ursache für das unbeabsichtigte Öffnen der Lider unter dem Verband ist eine unwillkürliche Levatorkraft, die von der vertikalen Blickrichtung des sehenden (und des verbundenen) Auges abhängt. Unter Verbänden und Uhrglasverbänden steigt die Temperatur der Luft um rund 8,0 °C an. Die Haut wird um 2,0 °C erwärmt. Der wärmende Effekt beider Verbandarten unterscheidet sich nicht signifikant. Bei Augengesunden ist die Luft unter Uhrglasverbänden mit Wasser gesättigt, unter herkömmlichen Gaze/Watte-Verbänden steigt die relative Luftfeuchtigkeit nur auf ca. 75%. Bei extrem trockenen Augen ist der feuchthaltende Effekt eines Monoculus nicht größer als der einer herkömmlichen Brille in Verbindung mit einem Augentropfen pro Stunde. Unter Uhrglasverbänden wird nur dann Sättigung mit Wasser erreicht, wenn wesentliche Hornhautoberflächendefekte vorliegen oder vor Anlegen des Verbandes ein Augentropfen angewendet wird. Wasserdichte Verbände, oder dicht sitzende Seitenschutzbrillen sind in Fällen von Metaherpes und gleichzeitig vorliegender Benetzungsstörung nicht nur Therapie, sie haben auch diagnostischen Wert, weil der Behandlungserfolg rückblickend die Schwere der Benetzungsstörung erkennen läßt.
Summary
The common monocular eye pad does not close the lids. The cause for the unintentional opening of the lids under dressing is a conjugate levator force depending on the elevation of the looking (and the dressed) eye. Eye pads and elastic dressings with transparent shields increase the temperature of the air below them by about 8 °C, of the skin by 2 °C. The warming effect of both bandages does not differ significantly.
Occlusive dressings saturate the air over normal eyes with water, under the common pads the relative humidity increases only to 75%. In extreme dry condition the moisturizing effect of eye pads is not higher than that of usual glasses, when the patient applies one eye drop per hour. Even under water-tight dressings saturation is reached only if the cornea has major erosions or an eye drop is given before dressing. In case of metaherpetic combined with dry eye disease waterproof bandages and goggles are of diagnostic value too, because their therapeutic effect shows retrospectively the degree of dehumidiflcation involved.
