Sympathische Ophthalmie

Zusammenfassung

Die SO, eine heute seltene Erkrankung, kommt ebenso häufig nach perforierenden Verletzungen wie nach intraokularen Operationen vor. Das klinische Bild der diffusen Panuveitis ist oft (60%) kombiniert mit einer serösen Amotio an einem oder beiden Augen.

Die Pathogenese bleibt nach wie vor unklar, wenn auch sehr vieles (experimentelle Arbeiten und neuere Therapieerfolge mit Zytostatica) für eine Autoimmunerkrankung spricht. Neuerdings wird eine Polypeptidfraktion der Sinnesepithel-außenglieder als mögliches Antigen (Retinal S-Antigen) diskutiert.

Die Therapie muß eine immunsuppressive sein, wobei nach unserer Erfahrung der Kombination Endoxan-Natulan gegenüber anderen Zytostatica der Vorzug zu geben ist. Kortison allein genügt nicht.

Summary

The clinical picture of sympathetic ophthalmia is discussed on the basis of the cases seen at the Department of ophthalmology during the last years. Inflammatory alterations can be seen in the anterior segment, but more often in the choroid, leading to an exsudative retinal detachment. Perforating injuries and intraocular surgery are equally important as pathogenetic factors. The etiology of sympathetic ophthalmia remains unknown, although recent experimental work points to a possible role of the so-called retinal S antigen from rods outer segments.

The possibility of immunosuppressive treatment and the importance of the enucleation of the sympathizing eye are being presented.
