Zusammenfassung
Die ätiologische Diagnose der Uveitis stellt für den Ophthalmologen ein besonderes Problem dar. Die Ursache dafür liegt einerseits in der Natur der Erkrankung, zum anderen in der großen Zahl der zur Verfügung stehenden Untersuchungs- und Test-Methoden. Darüber hinaus mag der Wert jeder ätiologischen Diagnostik in Zweifel gezogen werden, da in vielen Fällen der Einsatz von Kortikosteroiden zu einer scheinbaren Ausheilung führt, auch wenn die Ursache des Krankheitsprozesses unklar bleibt.
Dennoch ist eine gezielte Diagnose notwendig, um ätiologische Faktoren so weit wie möglich aufzudecken, um möglichst eine entsprechend gezielte Therapie einzuleiten und Anhaltspunkte für die Prognose zu erhalten.
Man sollte dabei von einer genauen pathologisch-klinischen Klassifizierung des Einzelfalls ausgehen und die durchzuführende Untersuchung am jeweils vorliegenden Uveitis-Typ orientieren (Ausschluß-Diagnostik). Dabei müssen auch anamnestische Daten berücksichtigt werden. Besondere Aufmerksamkeit ist der Aussagekraft der unterschiedlichen Testverfahren zu widmen, um Fehlinterpretationen zu vermeiden.
Summary
The etiologic diagnosis of clinical uveitis presents a special problem. This is due to the nature of the disease and the great number of laboratory tests that may be employed.
Nevertheless a systemic and organized approach is necessary to successfully manage such patients. The basis must always be the meticulous evaluation of the patient’s physical findings supplemented by anamnestic data.
From this one may establish a list of likely etiologic diagnosis and order laboratory tests to rule them out.
