Zusammenfassung
Die Auswertung der Krankenblätter von 345 Patienten mit schwerer rezidivierender, klinisch behandelter Iridozyklitis zeigt, daß in etwa 20% der Fälle der pathologische Ausfall der serologischen Reaktionen auf Leptospirose-, Lues-, Toxoplasmose- und Streptokokken-Infektionen den einzigen Hinweis für eine mögliche Ätiologie der Iridozyklitis ergibt.
Man sollte bei der Routinediagnostik nicht auf die Antikörperbestimmung gegen Leptospirose verzichten, auch wenn bei der Seltenheit des positiven Titers (1,2%) die Kostennutzenrelation ungünstig zu liegen scheint. Gerade durch den serologischen Hinweis ist in diesen Fällen die gezielte weitere Abklärung und Therapie möglich.
In der Altersgruppe der bis 14jährigen bot nie die Serologie allein einen Hinweis für eine Iridozyklitis-Ätiologie. Bei über 15 Jahre alten Patienten rechtfertigt die unterschiedliche Häufigkeit der positiven Serumreaktionen für Toxoplasmose und Streptokokken in den verschiedenen Altersgruppen nicht, im Einzelfall auf eine routinemäßige serologische Abklärung zu verzichten.
Summary
The evaluation of 345 case reports of patients with chronical recurrent iridocyclitis shows that in about 20% of the cases the serological reactions against leptospirosis, lues, toxoplasmosis and streptococcus-infections were the only indication of a potential aetiology of the iridocyclitis.
For children up to 14 years the serological reactions were never the only indication for the aetiology of the iridocyclitis. The clinical finding was important as well. In spite of the different frequency of the serological reaction against toxoplasmosis and leptospirosis in the different groups of age in patients over 15 years one shouldn’t resign to look for them as a matter of routine.
