Zusammenfassung
Beim stenosierenden, inoperablen Carcinom des Oesophagus, der Kardia und des oberen Magenpoles ist das Anlegen einer Ernährungsfistel häufig die letzte ärztliche Maßnahme. Es wird zwar dadurch der Inanitionstod des Kranken verhindert, sein Zustand damit aber nicht lebenswerter, da das Fehlen des physiologischen Schluckaktes eine schwere psychische Belastung darstellt. Berücksichtigt man dazu noch die kurze Lebenserwartung und die nicht unerhebliche Mortalität der Witzel-Fistel (14,5–43% nach Grewe, Cooper u. Mitarb., Ungeheuer, Gütgemann u. Mitarb.) bei diesen meist stark reduzierten Patienten, so ist man berechtigt, palliative Maßnahmen zu benützen, die eine annähernd normale Schluckfähigkeit wiederherstellen. Eine Umgehungsanastomose ist trotz einiger guter funktioneller Resultate wegen der Größe des Eingriffes nicht mehr gerechtfertigt, wenn mit der einfacheren und risikoloseren Methode der Tumorintubation mittels einer Endoprothese ein gleiches Resultat erzielt werden kann.
