Zusammenfassung
Die organischen Nitrate Glyceroltrinitrat (Nitroglyzerin, GTN) und Pentaerithrityltetranitrat* (PETN) werden häufig bei der Therapie verschiedener kardiovaskulärer Krankheiten als Vasodilatatoren angewandt. Die antiischämische Therapie mit organischen Nitraten wird jedoch durch die nitratinduzierte Toleranz erheblich beeinträchtigt. Die Metabolisierung der Nitrate mit anschließender NO-Freisetzung verursacht nämlich eine beträchtliche Koproduktion von Sauerstoffradikalen, die zu einer mehr oder weniger starken Inaktivierung des gerade freigesetzten NO führen. Über eine nitratinduzierte Erhöhung der reaktiven Sauerstoffspezies ROS, wie z. B. Superoxid-Radikal oder Peroxynitrit (ONOO−) in vivo liegen bisher keine Untersuchungen vor. In dieser Arbeit haben wir deshalb die verstärkte in-vivo-ROS-Bildung untersucht, die bei der Behandlung mit GTN, weniger mit PETN beobachtet wird. Außerdem haben wir die günstigen, radikalabfangenden Wirkungen von gleichzeitig verabreichten Antioxidantien, wie z. B. Vitamin C, dokumentiert. Die ROS-Bildung wurde mit Hilfe der Elektron-Spin-Resonanz-Spektroskopie gemessen [Oxidation von 1-Hydroxy-3-carboxy-pyrrolidin (CP-H) zu dem paramagnetischen 3-Carboxyproxyl (CP)]. Dazu erhielten anästhesierte Kaninchen 9 mg/kg CP-H als Bolus und anschließend 225 µg/kg/min CP-H als i. v. Infusion sowie 130 µg/kg GTN bzw. PETN als Bolusinjektion. Alle 5 Minuten wurden 200 µl Blutproben von der A. carotis entnommen, um die CP-Bildung zu messen. Die CP-Radikalbildung als Maß für die ROS-Bildung im Blut nach Infusionen betrug unter Kontrollbedingungen 0,2±0,01 µM, unter der Gabe von GTN 2,0±0,4 µM bzw. unter der Gabe von PETN 0,98±0,23 µM. Eine Vorbehandlung mit 30 mg/kg Vitamin C (Vit-C) führte zu einer signifikanten Reduktion der CP-Bildung: 0,27±0,14 µM (Vit-C+GTN) und 0,34±0,15 µM (Vit-C+PETN).
Demnach beobachten wir nach einer in-vivo-Infusion von GTN oder PETN eine Erhöhung der ROS-Bildung in vivo. PETN verursachte aber nur eine minimale Stimulierung der ROS-Bildung ohne gleichzeitige Entwicklung von Nitrattoleranz, wobei PETN sowie GTN vergleichbare vasodilatatorische Wirkungen zeigten. Unsere Ergebnisse deuten darauf hin, daß die durch organische Nitrate induzierte ROS-Bildung mit der Entwicklung von Nitrattoleranz eng korreliert ist. Die Wirkung von Vitamin C auf CP-Radikal- bzw. Superoxid-/Peroxynitrit-Bildung läßt den Schluß zu, daß Vitamin C als Antioxidans wirksam gegen nitratinduzierte ROS-Bildung in vivo schützt und damit die verschiedenen NO-Donatoren in ihrer Wirkung deutlich verbessert.
