Total Correction of Complete Atrioventricular Canal: Surgical Technique and Analysis of Long-Term Results

Summary

Surgery for total correction of complete AVC can be done with low early and late mortality. The operative risk is high only in patients who reach the operating room in a near moribund condition (NYHA V). There is clinical evidence that early operation — before the onset of pulmonary vas-cular disease — will further lower early and late mortality. The surgical technique has been refined to avoid late reoperation for left atrioventricular valve incompetence. The early and late results of operative therapy compare favourably with the natural history of this complex con-genital cardiac disease.

Zusammenfassung

Die operative Behandlung des kompletten AV-Kanals kann mit niedriger Früh- und Spätletalität durchgeführt werden. Das Operationsrisiko ist nur bei den Patienten hoch, die den OP in nahezu moribundem Zustand erreichen. Klinische Erfahrungen zeigen, daß eine frühe Operation — vor Einsetzen einer pulmonalen Gefäßkrankheit — die Früh- und Spätletalität weiter senkt. Die chir-urgische Technik wurde soweit verfeinert, daß eine Spätoperation wegen Insuffizienz der linken AV-Klappe vermieden wird. Früh- und Spätergebnisse sind günstig im Vergleich zum natür-lichen Verlauf dieses höchst komplexen kongenitalen Vitiums.

Résumé

L’opération pour corriger le canal atrio-venticulaire peut être effectuée avec un taux de mortalité précoce et tardive relativement bas. Le risque opératoire est important uniquement dans les cas moribonds pré-opératoires. Les données cliniques démontrent que, l’opération étant réalisée le plus tôt possible, c.â.d, avant l’apparition d’une hypertension pulmonaire, le taux de mortalité précoce et tardive en est d’autant plus bas. La technique opératoire a été perfectionnée â un tel point qu’elle permet d’éviter une éventuelle reoperation ä cause d’un fonctionnement insuffisant de la valve gauche atrio-ventriculaire. Les résultats obtenus ä court et â long terme montrent, en comparaison avec l’évolution naturelle de cette maladie congenitale très complexe, les avantages de l’intervention chirurgicale.
