Deep Anterior Resection with Circular Stapled Anastomosis of Congenital Megacolon: Clinical Results

Summary

This report deals with instrumental suture in the deep anterior rectum resection when treating Hirschsprung’s disease. The concept includes: ensuring diagnosis, cleaning the intestines, anti-biotic prophylaxis, intraoperative testing of the anastomosis and postoperative X-ray by a stan-dardized technique. Our first experience in the use of the stapler was gained with ten patients. One case of anastomosis insufficiency occurred. With the stapler apparatus, safe anastomosis was possible. The stapler facilitates anastomoses in deep resections. Therefore, a deep resection of the pathological segment in Hirschsprung’s disease is possible. The practicality of the stapler and the favourable clinical experience justify the continued application of the instrumental suture technique in children’s surgery.

Zusammenfassung

Es wird über die Instrumentenanastomose zur tiefen anterioren Rektumresektion bei der Be-handlung des M.Hirschsprung berichtet. Das Behandlungskonzept umfaßt Diagnosesicherung, Darmspülung, Antibiotikaprophylaxe, intraoperative Testung der Anastomosensuffizienz und postoperative standardisierte Röntgenkontrolle der Anastomose. Erste Erfahrungen mit dem EEA-Stapler wurden bei 10 Patienten gesammelt. Dabei trat 1 Fall von Anastomoseninsuffizienz auf. Eine sichere Anastomosierung mit dem Stapler war möglich. Der Stapler erleichtert die Anastomose bei der tiefen Resektion, so daß das aganglionäre Segment beim M.Hirschsprung sehr tief abgesetzt werden kann.

Praktikabilität und gute klinische Ergebnisse lassen die klinische Anwendung der Instrumen-tenanastomose auch bei Kindern angeraten erscheinen.

Résumé

Nous rapportons les cas d’anastomose lors d’une résection rectale profonde au décours d’une maladie de Hirschsprung. La thérapeutique comprend: une confirmation du diagnostic, un lave-ment, une antibiothérapie de protection, une vérification per- et post-opératoire de la solidité des sutures et un contrôle radiologique standard de l’anastomose. Les premières expériences avec le “EEA stapler” ont été effectuées sur 10 patients. Dans un cas, nous avons eu un défaut de suture. La sûreté de la suture a été rendue possible grâce au stapler. Le stapler facilite les sutures dans les cas de résections larges, de telle sorte que le segment aganglionnaire de la maladie de Hirsch-sprung peut être largement réséqué.

L’utilisation partique et les bons résultats cliniques justifient l’anomastose également en chir-urgie infantile.
