Zusammenfassung
Ein neuartiges optisches Meßverfahren, das Erlanger Mikro-Lichtleiter-Spektrophotometer EMPHO I, ermöglicht auf der Basis der Remissionsspektroskopie die direkte und nicht-invasive Erfassung wichtiger kapillärer Versorgungsparameter, wie intrakapilläre Hämoglobinoxygenierung und -konzentration an lebenden Geweben. Der Einsatz hochflexibler Mikrolichtleiter und die hohe Aufnahmerate gestatten eine einfache Ankopplung auch an bewegte Organe. An der menschlichen Haut werden Signale aus der oberen Dermis innerhalb eines Versorgungsbereichs von etwa 4×4 Kapillaren empfangen.
Mit diesem Gerät wurden einige grundlegende Aspekte der kapillären Sauerstoffversorgung an der gesunden menschlichen Haut in vivo erarbeitet.
Es zeigte sich, daß die sog. Grundremission des blutleeren Hautgewebes durch die Absorption an den epidermalen Pigmenten Melanin oder Bilirubin beeinflußt wird. Im Falle einer starken Pigmentierung muß für eine exakte Auswertung der Gewebespektren nach Oxygenierungsgrad und Konzentration des intrakapillären Hämoglobins diese Grundremission berücksichtigt werden.
Bei Untersuchungen an hautgesunden Probanden wurden abhängig von der Region sehr unterschiedliche intrakapilläre Oxygenierungen gefunden, die maßgeblich von den lokalen Heterogenitäten der Kapillardurchblutung bestimmt waren. Am Innenknöchel wurden stellenweise Oxygenierungen von nur 20% gemessen, die für eine grenzwertige kapilläre Sauerstoffversorgung dieses Hautgebietes schon beim Gesunden sprechen. Störungen der Kapillardurchblutung, wie sie z. B. im Rahmen einer chronisch venösen Insuffizienz vorkommen, können dann frühzeitig zu einer Mangelversorgung solcher Hautbezirke führen. Der zeitliche Verlauf von Oxygenierungsgrad und Konzentration des intrakapillären Hämoglobins während eines arteriellen Durchblutungsstopps oder einer lokalen Erwärmung der Haut verdeutlicht lokale Regulationsmechanismen, die das wirksame Zusammenspiel von Sauerstoffangebot, -verbrauch und lokaler Durchblutung auf kapillärer Ebene steuern.
Summary
A new instrument, the Erlangen Micro-Lightguide Spectrophotometer EMPHO I, enables via remission spectroscopy the direct and non-invasive determination of important capillary supply parameters in vital tissues. These are in particular the oxygenation and concentration of intracapillary hemoglobin. Highly flexible microlightguides and the high sampling rate allow also an easy link-up to organs in motion. On human skin, EMPHO I receives signals (spectra) from the upper dermis within a supply area of approximately 4×4 capillaries.
We report on some basic aspects of remission spectroscopy on human skin in vivo investigated with EMPHO I.
The so-called basic remission of bloodless skin tissue is influenced by absorption on the epidermal pigments melanin and bilirubin. In coloured tissue, this basic remission has to be considered for exact evaluation of the remission spectra.
In healthy skin, major regional differences in oxygenation of intracapillary hemoglobin were found. They are strongly determined by the local heterogeneities of capillary perfusion. Values of only 20% oxygenation were measured at the malleolus medialis, indicating a borderline oxygen supply of this skin area even in healthy subjects. Disturbances of capillary perfusion occurring in cases of chronic venous insufficiency may cause an early deficiency in oxygen supply of such areas.
The time course of oxygenation and concentration of intracapillary hemoglobin during an occlusion reaction or while heating the skin, points to local regulatory mechanisms that control the interaction between supply and consumption of oxygen and the local perfusion in the cutaneous capillary system.
