Zusammenfassung
Frakturen des oberen Sprunggelenks sind mit einer Inzidenz von 1,14–1,87‰ (Lindsjö 1985, Daly et al. 1987) wahrscheinlich die am häufigsten behandelten Brüche (Praemer 1992). Sie betreffen die distale Fibula, den Innenknöchel und die gelenkbildende Tibia. Begleitend können die Syndesmose, die Membrana interossea, das Deltaband und der Kapselapparat verletzt sein (Segal und Leach 1983). Als Folge einer indirekten Gewalteinwirkung wird die Kraft bei fixiertem Fuß als Adduktion, Abduktion oder Rotation des Talus auf Tibia und Fibula übertragen. Diese Verletzungsmechanismen verursachen typische Frakturformen, die anatomisch oder nach Entstehungsmechanismus eingeteilt werden können. Frakturlokalisation, Grad der Instabilität und Dislokation, Reponierbarkeit und Retinierbarkeit sowie lokale und allgemeine Faktoren entscheiden über die Indikation zur operativen Stabilisierung.
