Zusammenfassung

Hyperammoniaemien finden sich, wie Herr Gerok eben ausgeführt hat, vorwiegend bei Lebererkrankungen und hier besonders bei fortgeschrittenen Cirrhosen. Die Hyperammoniaemie ist dabei durch eine Störung der Ammoniakentgiftung in der Leber, sicherlich aber auch durch eine gesteigerte Ammoniakbildung beim Zellzerfall und durch Veränderungen der Bakterienflora im Darm bedingt. Weiterhin ist ein wesentlicher Faktor für die Hyperammoniaemie der Cirrhosekranken in zirkulatorischen Veränderungen zu sehen. Sobald eine Cirrhose zur portalen Hypertonie und damit zur Ausbildung eines mehr oder minder stark ausgeprägten Kollateral-kreislaufes geführt hat, wird das ammoniakhaltige Pfortaderblut unter Umgehung der Leber über diese Kollateralgefäße direkt in die großen Körpervenen abgeleitet.
