Zusammenfassung
Unter den ACTH-abhängigen Varianten des Cushing-Syndroms hat das von Liddle u. Mitarb. (11) sog. ektopische ACTH-Syndrom an Interesse gewonnen. Es beinhaltet die Abhängigkeit des Rindenüberfunktionszustandes von einem extrahypophysären corticotrop aktiven Tumor. Die Erstbeschreibung des Syndroms erfolgte durch Brown (1) 4 Jahre vor der Arbeit von Cushing (3) über den hypophysären Basophilismus. Seine Häufigkeit läßt sich nur grob abschätzen. McCullagh (4) sah in seinem Patientengut vier unter insgesamt 85 Fällen mit Cushing-Syndrom (5%); Ross (15) gibt 2% an. Wir verfügen über zwei Fälle unter insgesamt 37 Fällen von M. Cushing und Cushing-Syndrom im Erwachsenenalter (Operations- und Sektionsmaterial). Dies entspricht einem Prozentsatz von 4,5%, der sich jedoch auf 12,5% erhöht, wenn nur das Sektionsmaterial von Cushing-Syndrom zugrunde gelegt wird (16 Fälle) bzw. 22% beträgt, wenn man die Werte lediglich auf Sektionsfälle mit bilateraler Nebennierenrindenhyperplasie bezieht. Hieraus resultiert, daß das ektopische ACTH-Syndrom zwar selten ist, aber anteilsmäßig an einem Cushing-Kollektiv nicht unterschätzt werden sollte. Aus der Literatur konnten wir bis Ende 1965 180 Fälle mit ektopischem ACTH-Syndrom zusammenstellen. Hierbei wurden erstens die klinische Symptomatik, Elektrolytstoffwechselstörungen, besonders hypokaliämische Alkalose, diabetische Stoffwechsellage und Angaben über den Steroidstoffwechsel, zweitens Tumorlokalisation und Typ, Nebennierengewicht, Rindenstruktur und Nachweis von Crooke-Zellen im Hypophysenvorderlappen sowie drittens Befunde über den biologischen und immunhistochemischen ACTH-Nachweis im Tumorgewebe herangezogen.
