Zusammenfassung
Die sicherste Therapie erworbener Lederschäden besteht in der Ausschaltung der verursachenden Noxe [28], wofür der alkoholische Leberschaden ein Schulbeispiel ist. Die therapeutische Wirkung der Alkoholkarenz wurde deutlich durch unfreiwillige Massenversuche, wie es die Prohibition in den USA, die Verteuerung alkoholischer Getränke durch Wirtschaftskrisen oder Alkoholsteuern [23, 26] und die Weinrationierung in Paris während der beiden Weltkriege [19] waren, oder wie es histologische Studien mit Hilfe von Serienbiopsien belegen konnten [29]. Auch schwerste alkoholische Leberverfettungen verschwinden unter Alkoholkarenz innerhalb von 2 bis 4 Wochen und alkoholische Hepatitiden heilen, falls der Zerstörungsprozeß nicht zu weit fortgeschritten ist, nach 6 bis 12 Wochen unter Hinterlassung belangloser Narben aus [29]. Galambos [17] berichtet zwar über wenige Fälle, bei denen der anatomische Krankheitsprozeß trotz Alkoholentzug weiter fortzuschreiten schien, ein Ereignis, das wir in unserem großen Krankengut von über 4000 Patienten nur dann beobachten konnten, wenn der alkoholische Leberschaden durch eine chronische Virushepatitis kompliziert war oder — was viel häufiger der Fall ist — die Alkoholkarenz nur vorgetäuscht war [29]. Galambos ist aber dahingehend beizupflichten, daß kleine Alkoholdosen den Heilungsprozeß nicht beeinträchtigen. Trotzdem ist beim Alkoholkranken die absolute Alkoholkarenz die Voraussetzung jeder erfolgreichen Behandlung. Kompromisse führen fast unweigerlich dazu, daß der Kranke die Anweisungen seines Arztes nicht ernst genug nimmt und über kurz oder lang wieder dem Abusus verfällt.
