Zusammenfassung
Die bahnbrechende Neuerung der letzten 15 Jahre für die Diagnostik in der Neurologie betrifft die Einführung und breite klinische Verfügbarkeit dreidimensionaler bildgebender Verfahren, wobei die neueren und personell und verfahrenstechnisch aufwendigeren Methoden auch die Beurteilung funktioneller Aspekte des normalen und pathologisch veränderten Hirngewebes erlauben. Mit der Einführung der Röntgen-Computer-Tomographie (CT) durch Hounsfield [1973] wurde die Diagnose neurologischer Krankheitsbilder revolutioniert. Im Gegensatz zur konventionellen Röntgendiagnostik — bei der übereinanderliegende Strukturen eines voluminösen Organs in einer Ebene dargestellt werden und normales und pathologisches Gewebe im Gehirn üblicherweise nicht differenziert werden kann — werden bei der CT aus vielen Projektionen die Röntgenabsorption kleiner Gewebsvolumina im Computer errechnet und Schnittbilder des Schädels und des Gehirns rekonstruiert, auf denen die anatomischen Strukturen des Gehirns erkannt und morphologische Veränderungen abgegrenzt werden können. Die CT mißt Dichteunterschiede im Gewebe und bildet auf mehreren Schnitten dreidimensional die Morphologie ab. Sie ist damit in der Lokalisationsdiagnostik von Läsionen und gestörter Funktionen den neuropathologischen Methoden vergleichbar, kann aber im Gegensatz zu diesen ohne Belastung wiederholt durchgeführt werden, so daß Beziehungen zwischen Entwicklung neurologischer Ausfälle und Änderung in der Ausdehnung und Art von lokalen Hirnschädigungen dargestellt werden können.
