Zusammenfassung
Die Forschung der letzten Jahre hat wesentlich zur Erhärtung der Annahme beigetragen, wonach die Hyperglykämie, d.h. das erhöhte Angebot von Glukose, vorrangig für die Entstehung der diabetischen Spätkomplikationen verantwortlich ist. Dabei kann man heute formal intrazelluläre, strukturelle Veränderungen infolge erhöhter Glukosekonzentrationen unterscheiden (Tabelle 1). Die ersteren betreffen die insulinunabhängigen Gewebe wie Niere, Retina, Nerven, Augenlinse — also die Prädilektionsorte von Mikroangiopathie and Neuropathie ä, und sind durch vermehrte Bildung von Sorbit bei gleichzeitiger Verminderung des myoInositgehaltes der Zellen charakterisiert. Die letzteren betreffen die extrazelluläre Matrix, wo es infolge nichtenzymatischer Glykolysierung der langlebigen Proteine, insbesondere des Kollagens, zur Struktur- and Funktionsänderungen kommt.
