Zusammenfassung
Die Transfusion hämatopoetischer Stammzellen in einen in geeigneter Weise konditionierten und selektierten Empfänger kann im Erfolgsfall in wenigen Tagen bis Wochen zu einer vollständigen Neubildung der Blutzellbildung und der Immunkompetenz führen [1–5]. Werden die Stammzellen aus dem Knochenmark gewonnen, so spricht man aus historischen Gründen von einer „Knochenmark“-Transplantation [6]. Pathophysiologisch betrachtet handelt es sich jedoch nicht um eine Organtransplantation, wie sie als Hauptthema in diesem Kongreß behandelt wurde. Vielmehr wird nicht das Gesamtorgan, sondern nur ein einziges zelluläres Element — die Stammzellen — benötigt [7]. Das Organ Knochenmark besteht aus dem Stroma einerseits und dem Parenchym andererseits, also aus Gefäßsystem, Nerven und Retikulumzellen und den hämatopoetischen Zellen bzw. Zellsystemen (Abb. 1). Die funktionsfähige Matrix — das Stroma — ist eine notwendige und unabdingbare Voraussetzung für das Fließgleichgewicht der Blutzellbildung [8,9]. Der Begriff „Knochenmarktransplantation“ bezieht sich also auf die Tatsache, daß Stammzellen aus dem Knochenmark gewonnen und transfundiert werden und sie sich in einer geeigneten Empfängermatrix ansiedeln, aus bestimmten entwicklungsgeschichtlichen Gründen vorzugsweise im Knochenmark [10].
