Zusammenfassung
Das intakte Gefáßendothel trägt durch zahlreiche Mechanismen zur Aufrechterhaltung der Homöostase des Blutgerinnungssystems bei. So vermögen Endothelzellen Thrombin durch Bindung an den Endothelrezeptor Thrombomodulin zu inhibieren. Thrombin bildet mit diesem spezifischen Rezeptor einen 1:1 molaren, hochaffinen Komplex (Kd: 0,48 nM) (Esmon u. Owen 1981) und ändert dadurch seine Konformation und seine Aktivität: An Thrombomodulin gebundenes Thrombin verliert einerseits die Fähigkeit, die Gerinnung durch Aktivierung der Blutplättchen zu stimulieren (Esmon etal. 1983) sowie den Faktor V und Fibrinogen zu spalten (Esmon et al. 1982 a), vermag aber andererseits ca. 20 000fach schneller als vor Bindung an Thrombomodulin zirkulierendes Protein C (PC) zu aktivieren (Esmon et al. 1982 b). Aktiviertes PC (APC) ist eine Serinprotease, welche durch proteolytische Spaltung die Faktoren Va und VIIIa inaktiviert (Abb. 1) und demnach als ein Inhibitor der Blutgerinnung wirkt (Walker et al. 1979, Fulcher et al. 1984). PC wird in der Leber Vitamin K-abhängig synthetisiert. Bei Patienten sowohl mit angeborenem als auch mit erworbenem PC-Mangel sind gehäuft Thrombosen und Embolien beobachtet worden (Bertina et al. 1982, Griffin 1984).
