Zusammenfassung
1978 berichteten Todd et al. über eine akute Erkrankung, die sie bei sieben Kindern zwischen Juni 1975 und November 1977 beobachtet hatten [16]. Das jüngste Kind war 8 Jahre, das älteste 17 Jahre alt. Die Erkrankung begann in allen Fällen akut, mit Temperaturen zwischen 39° C und 41° C, Kopfschmerzen, Verwirrtheit, diffusem, nicht juckendem Exanthem, Halsschmerzen, Erbrechen, Durchfällen, Konjunktivitis, Pharyngitis, Ikterus, Oligurie sowie Schock. In den Laboratoriumsuntersuchungen fielen eine Leukozytose mit Linksverschiebung, Thrombopenie, Bilirubin-, SGOT-, Harnstoff- und Kreatininerhöhung auf. Bei sechs dieser Kinder wurde S. aureus Phagotyp I isoliert. Im Laufe der Erkrankung fiel eine Desquamation der Gesichts-, Stamm-sowie Extremitätenhaut auf. Betont war vor allem die Desquamation der Palmar-bzw. Plantarhaut. Todd benutzte in seinem Bericht erstmalig den Begriff „Toxic-Schock-Syndrome“ (TSS) [16]. Er wies auch auf den Zusammenhang zwischen der Erkrankung und den isolierten Staphylokokken hin. Als nächstes berichteten Shands et al. 1980 [10] sowie Tofte und Williams 1981 [17] über 52 bzw. 36 Patientinnen mit gleichem Krankheitsbild. Allen diesen Fällen war das Auftreten der Erkrankung während der Menstruation gemeinsam. Die von Shands et al. [10] und Tofte und Williams [17] beobachtete klinische Symptomatologie war mit der von Todd et al.]16] beobachteten identisch (Tabelle 1). Im Mai 1981 publizierten Centers for Disease Control, Atlanta (USA) die Ergebnisse einer epidemiologischen Studie über 55 Patienten, von denen 52 (= 95%) Frauen waren [4]. Hierbei fiel auf, daß die meisten Frauen die Erkrankung während der Menstruation entwickelten, und daß das Risiko TSS zu entwickeln bei den Frauen am größten war, die eine bestimmte Tamponmarke benutzten [4]. Eine spätere Untersuchung von Harvey et al. konnte die Rolle der Tampons nicht eindeutig wissenschaftlich belegen [6]. Seit den ersten Beobachtungen nahm in den USA bis 1980 die Anzahl der gemeldeten Erkrankungen zu, um dann bis 1982 wieder spontan abzunehmen. Über Krankheitsfälle ist auch aus anderen Ländern, unter anderem aus Deutschland, berichtet worden [2, 5, 11–13, 18].
