Therapie der Ösophagusvarizenblutung

Zusammenfassung

Die portale Hypertonie gleich welcher Genese führt zur Ausbildung verschiedenster Gefäßverbindungen zwischen der portalen und der systemischen Zirkulation. Solange der Weg dieser Anastomosen nicht über Ösophagus und Magen führt, verursachen sie nur Enzephalopathie. Liegen die Kollateralgefäße aber in der Submukosa des Ösophagus oder des Magens, dann besteht ein hohes Risiko der Ruptur. Dieses Risiko der Ruptur ist wiederum wesentlich größer bei Ösophagusvarizen als bei Varizen im Magenfundus. Annähernd ein Drittel aller Patienten mit Leberzirrhose erleidet eine oder mehrere Ösophagusvarizenblutungen [1].
