Zusammenfassung

Lebererkrankungen weisen in fortgeschrittenen Stadien eigentümliche funktionelle Nierenveränderungen ohne morphologisches Korrelat auf. Sie sind durch eine verminderte glomeruläre Filtrationsrate, Anstieg der harnpflichtigen Substanzen im Blut, gesteigerte Natriumreabsorption und Oligurie mit Ausscheidung eines konzentrierten Urins [7] gekennzeichnet. Wiederholt wurden als Erklärung hierfür intrarenale Durchblutungsstörungen verantwortlich gemacht [2, 9]. Welche vasoaktiven Wirkstoffe kausal beteiligt sind, ist umstritten. Messungen der Noradrenalinkonzentration im Nierenvenenblut spiegeln nach Tierversuchen [4, 6] den renalen Sympathikotonus wider. Mit ansteigender elektrischer Reizfrequenz stieg bei anästhesierten Hunden die Noradrenalinkonzentration im renalvenösen Blut linear an. Bei fortgeschrittenen chronischen Lebererkrankungen ist, mit neueren Methoden gemessen, nunmehr zweifelsfrei, daß die Noradrenalinkonzentration im systemischen Kreislauf erhöht ist [3, 10].
