Zusammenfassung
Bei Untersuchungen über die Pathogenese der Hypertonic sind in den letzten Jahren enge Beziehungen zwischen Blutdruckregulation und Volumenhomöostase in den Vorder-grund gestellt worden [6,10]. Dem Blutdruckregulationskonzept von Guyton [6] zufolge wird eine Salz-Flüssigkeitsbelastung via Blutdruckerhöhung über die Niere eliminiert. Blutdruckanstiege unter einer akuten intravenösen Kochsalzbelastung sind jedoch bei Versuchen an normotensiven und hypertensiven Patienten nur in einem geringen Prozentsatz beobachtet worden [1, 2, 14, 15]. Außerdem ist nach diesem Konzept die seit über 30 Jahren beschriebene exaggerierte Natriurese bei Hypertonikern [4] schwer zu verstehen, da dieses Phänomen der Hypertonieerhaltung entgegenwirken sollte [7]. In der folgenden Studie wird die Wirkung einer starken akuten Salz-Flüssigkeitsbelastung auf das Blutdruckverhalten, die intra-extravasale Flüssigkeitsverteilung und auf die renale Salz-W asserausscheidung bei normotensiven und hypertensiven Patienten untersucht. sucht.
