Zusammenfassung
Den Prostaglandin-Endoperoxyden PGG2 und PGH2 und insbesondere dem Folgeprodukt Thromboxan A2 wird eine wesentliche Funktion bei der Induktion der Thrombozytenaggregation und Plättchenthrombusbildung zuerkannt. Arachidonsäure wird durch die Phospholipase A2 aus der Thrombozytenmembran freigesetzt, durch die Zyklooxygenase in die Prostaglandin-Endoperoxyde PGG2 und PGH2 und weiter durch die Thromboxan A2-Synthetase in Thromboxan A2 umgesetzt [13]. Die Prostaglandin-Endoperoxyde und das Thromboxan A2 werden in der zweiten Phase der durch Adrenalin ausgelösten Aggregation gebildet. Daneben ist eine Stimulierung der Plättchen mit Freisetzungsreaktion auch unter Umgehung des Prostaglandin systems müglich [4, 7, 8, 9]. In der GefäÜwand von Arterien und Venen fmdet sich ein analoges Prostaglandinsystem. Auf entsprechenden Reiz wird Arachidonsäure auch hier durch die Zyklooxygenase in die Prostaglandin-Endoperoxyde umgewandelt. Der nachfolgende Schritt ist nun aber die Transformation Prostazyklin (PG I2) durch die Prostazyklinsynthetase. Prostazyklin wirkt aggregationshemmend und soll die Gefäßwand vor unkontrollierten Thrombozytenauflagerungen schützen [5, 11]. Die Zyklooxygenase beider Systeme wird durch Acetylsalicylsäure irreversibel acetyliert und inaktiviert. Die Zyklooxygenase der nach einmaliger Gabe von Acetylsalicylsäure gebildeten Thrombozyten wird nicht acetyliert und ist voll aktiv [3, 12, 15].
