Zusammenfassung
Bei 14 Patienten, die sich im Leberkoma Stadium IV befanden, wurde die Hämoperfusion über Aktivkohle eingesetzt. Bei neun Patienten war die Ursache des Leberkomas eine fulminante Virushepatitis und bei fünf Patienten eine Knollenblätterpilzvergiftung. Bei den Patienten mit fulminanter Hepatitis besserte sich das Komastadium bei 33%, während die Mortalität unbeeinflußt blieb. Bei Patienten mit Knollenblätterpilzvergiftung besserte sich das Komastadium bei 80%, während die Mortalität auf 40% gesenkt warden konnte. Durch die Hämoperfusion wurden neurotoxische Substanzen, wie die aromatischen Aminosäuren Methionin, Phenylalanin und Tryptophan sowie Merkaptanderivate eliminiert. Eine Korrelation zwischen der Konzentration im Plasma, der Eliminierung durch die Hämoperfusion und der Besserung der Komatiefe bzw. der Überlebensrate bestand nicht.
