Zusammenfassung

Klagt ein Patient über vorübergehende, an die Orthostase gebundene oder mit Armarbeit gekoppelte cerebrale Ausfälle allgemeiner Art wie Schwindel, Sehstörung, Ohnmacht, kann man wenigstens an einem Arm den Puls nicht oder nur mit Mühe fühlen und ist der Blutdruck nicht meßbar oder deutlich niedriger als auf der Gegenseite, dann sollte der Verdacht auf ein Aortenbogensyndrom [83], ein Verschlußsyndrom der Aortenbogenäste [64, 65] wach werden, für das diese Kombination einer cerebrovasculären Insuffizienz mit Durchblutungsstörungen der oberen Extremitäten typisch ist.

Die drei Etagen der arteriellen Hirnstrombahnen: 1 Distale Etage: Versorgerarterien (Aa. cerebri ant., med. und post; kleine Äste der A. carotis int. und der A. basilaris). Intra kranieller Verlauf. Funktionelle Endarterien. 2 Mittlere Etage: Cerebrale Zubringerarterien (A. carotis int., A. vertebralis, A. basilaris, Circulus Willisii). Teilweise intrakranieller, teil weise extrakranieller Verlauf. Verhalten sich bei Verschluß häufig nicht wie Endarterien, kollaterale Kompensation möglich. 3 Proximale Etage: Aortenbogenäste (Tr. brachio cephalicus = gemeinsamer cerebro-kraniobrachialer Zubringer, A. carotis com. = gemein samer cerebrokranialer Zubringer, A. Subclavia bis zum Abgang der A. vertebralis = gemein samer cerebrobrachialer Zubringer). Extrakranieller Verlauf. Verhalten sich bei Verschluß fast nie wie Endarterien. Kollaterale Kompensation in der Regel möglich
