Zusammenfassung
Seit Ende 1999 bzw. Mitte 2000 sind in Deutschland die selektiven COX-II-Hemmer Rofecoxib (für die symptomatische Behandlung der Arthrose) und Celecoxib (für Arthrose und zusätzlich rheumatoide Arthritis) zugelassen. In welchem Ausmaß diese im Vergleich zu den konventionellen nichtsteroidalen Antiphlogistika (NSA) kostenintensiveren Substanzen Vorteile bringen, ist bisher umstritten.
Als gesichert kann anhand zahlreicher kontrollierter Studien eine gegenüber Placebo überlegene und gegenüber einigen NSA (Diclofenac, Ibuprofen, Naproxen) äquivalente Wirksamkeit für beide Substanzen gelten. Ebenso haben eine Vielzahl von Studien Verträglichkeits-Vorteile aufgrund einer Senkung der Rate schwerer gastrointestinaler Komplikationen belegt; in zahlreichen kontrollierten Studien traten diese Komplikationen nicht häufiger als unter Placebo auf. Ein weiterer wichtiger Unterschied gegenüber konventionellen NSA ist der Wegfall einer Hemmung der Thrombozytenaggregation. Hieraus leitet sich als Vorteil ein geringeres Blutungsrisiko, als eventueller Nachteil aber möglicherweise der Wegfall einer Verringerung kardiovaskulärer Risiken ab. Ob dies von praktischer Relevanz ist, wurde bisher ebensowenig durch Studien geklärt wie die Frage, ob durch Zugabe von niedrig dosierter ASS als Thrombozytenaggregationshemmer bei Risikopatienten die gastrointestinalen Vorteile der Coxibe wieder eliminiert werden.
