Zusammenfassung
Nitrovasodilatatoren werden seit 100 Jahren in der Therapie der myokardialen Ischämie verwendet. Bei langfristiger, nicht-intermittierender Verabfolgung vasodi-latierender Substanzen im Rahmen der Angina pectoris führt Toleranzentwicklung gegenüber Nitraten oftmals zur Abschwächung des erwünschten kardialen Wirkungsprofils. Diese Toleranz stellt ein multifaktorielles Phänomen dar, dessen Analyse/Quantifizierung eine aufwendige Methodik erfordert. Nitrovasodilatatoren/NO-Donoren unterschiedlicher Struktur und Zusammensetzung sind mit jeweils substanzspezifischer Toleranzausprägung vergesellschaftet, die sich differentiell auf bestimmte Gefäßabschnitte erstreckt. Die meisten Nitrovasodilatatoren (z. B. Glyzeroltrinitrat, GTN) induzieren Toleranz innerhalb von 2–3 Tagen während einer nicht-intermittierenden Langzeittherapie.
