Zusammenfassung
Die Analgetika/Antirheumatika repräsentieren seit vielen Jahren die Indikationsgruppe mit der höchsten Verordnungshäufigkeit in der Bundesrepublik [1]. Obgleich diese Indikationsgruppe durch eine enorme Vielzahl von Arzneimittelspezialitäten und auch Wirkstoffen gekennzeichnet ist, besitzen die peripheren Analgetika oder die nichtsteroidalen Antiphlogistika doch in aller Regel einen gemeinsamen Wirkmechanismus, welcher in einer Hemmung der Zyklooxygenase und somit der Prostanoidbiosynthese besteht. Der grundlegende Mechanismus dieser Pharmakonklasse wurde erstmals von Smith und Willis [2] sowie von Vane [3] für das Aspirin beschrieben und später auch für verschiedene andere nichtsteroidale Antiphlogistika verifiziert [4]. Während die Bedeutung dieses Effektes für die pharmakotherapeutische Entzündungshemmung außer Frage steht, wurde doch mit der Entdeckung des 5-Lipoxygenase-Stoffwechselweges klar, daß auch andere Metaboliten des Arachidonsäure-Stoffwechselweges, wie beispielsweise das chemotaktisch wirksame Leukotrien (LT) B4 oder die vasokonstriktorischen und ödeminduzierenden Cysteinyl-LT, als wichtige proinflammatorische Mediatoren angesehen werden müssen. Um die Biosynthese von LT zu hemmen, sind in den letzten Jahren mit großem Aufwand verschiedene 5-Lipoxygenasehemmer — bislang allerdings mit relativ bescheidenem Erfolg — entwickelt worden [5].
