Zusammenfassung
Infektionen bilden auch heute noch das größte Mortalitätsrisiko schwerverbrannter Patienten. Die Letalität systemischer Infektionen erreicht 50-75%, bei Patienten mit Lungenversagen steigt die Letalität einer infektiösen pulmonalen Komplikation auf 90% [20, 21]. Gerade der Brandverletzte mit seinem in der Postaggresionsphase erheblich eingeschränkten Immunsystem ist den Gefahren einer bakteriellen Besie-delung besonders ausgesetzt. Bei dieser massiven Infektionsgefährdung der Patienten steht die Verhütung infektiöser Komplikationen neben einer frühzeitigen stabilen Hautdeckung im Mittelpunkt der therapeutischen Strategie. Auch der Einsatz anti-mikrobiell wirksamer Substanzen ist fester Bestandteil dieses Konzepts. Die Grenzen zwischen Indikation und Kontraindikation eines Antibiotikaeinsatzes erscheinen allerdings oft verwaschen, so daß die Rolle der Antibiotikatherapie beim Brandverletzten eine kritische Würdigung verdient [11, 20, 22, 29, 41].
