Zusammenfassung

Wenn man von perioperativer Infektionsprophylaxe spricht, denkt man in erster Linie an die Antibiotikaprophylaxe. Die perioperative Infektionsprophylaxe geht jedoch weit darüber hinaus. Trotz Antibiotikaprophylaxe finden sich häufig einige Keime, die sich in einer schlecht heilenden Wunde vermehren und sie infizieren können. Andererseits stören ein paar Keime mehr oder weniger die Heilung einer „gesunden“ Wunde nicht. Lokale und systemische Störfaktoren, die die immunologischen Wundheilungsvorgänge beeinträchtigen, sind dagegen von weit größerer Bedeutung. In dem bekannten Experiment von Elek (3) erhöhte ein Fremdkörper in Form eines Fadens die infizierende Wirkung eines Staphylokokkenstammes um das Hundertfache; das Knoten dieses Fadens in Form einer chirurgischen Naht, wodurch ein ischämischer Bezirk entsteht, erhöhte die infizierende Wirkung abermals um das Hundertfache.
