Summary
Whether cardiac glycosides, besides decreasing ventricular rate, are able to terminate atrial fibrillation or to prevent the recurrence of this arrhythmia is unclear at present. This action was studied in a randomized, prospective study in 45 patients with symptomatic paroxysmal atrial fibrillation to compare class I-antiarrhythmic drugs (group I (n=15): digoxin 0.5 mg orally per day; group II (n=15): digoxin 0.25 mg and quinidine hydrogensulphate 750–1000 mg; group III (n=15): digoxin 0.375 mg and flecainide 200–300 mg).
Over a mean follow-up of 29 weeks, complete suppression or a significant reduction of attacks of paroxysmal atrial fibrillation was achieved in only two patients in group I, compared to six patients of group II and 11 patients of group III; drug efficacy was not sufficient in 12 patients of group I, six patients of group II and four patients of group III. Drug action could not be classified in one patient of group I and in three cases in group II. Thus, efficacy of cardiac glycosides given alone was inferior to the additional administration of either quinidine or flecainide (p<0.05; Chi squared test and Fisher’s exact test). Side effects occurred in two cases in both group I and group III, compared to eight cases in group II.
Conclusions: (1) Oral long term therapy with cardiac glycosides is able to prevent paroxysmal atrial fibrillation in only a minority of cases (13%) and is significantly inferior to an additional therapy with either quinidine or flecainide; (2) While quinidine and flecainide appear to be equally effective, flecainide appears to be associated with fewer side effects. Results are preliminary because of a mean follow-up period of 29 weeks for this report.
Zusammenfassung
Ob Herzglykoside bei Vorhofflimmern neben der Senkung der Kammerfrequenz auch das weitere Auftreten dieser Rhythmusstörung selbst verhindern, ist unklar.
Diese Wirkung wurde randomisiert prospektiv an 45 Patienten mit symptomatischem paroxysmalen Vorhofflimmern im Vergleich zu Klasse-I-Antiarrhythmika untersucht (Gruppe I (n=15): Digoxin 5.mg per os täglich; Gruppe II (n=15): Digoxin 0,25 mg und Chinidinhydrogensulfat 750–1000 mg; Gruppe III (n=15): Digoxin 0,375 mg und Flecainid 200–300 mg).
Über einen mittleren Verlauf von 29 Wochen wurde eine vollständige Suppression oder deutliche Verminderung der Anfälle bei nur 2 Patienten der Gruppe I, dagegen 6 der Gruppe II und 11 Patienten der Gruppe III erzielt; eine nicht ausreichende Wirkung wurde 12mal in Gruppe 1,6mal in Gruppe II und 4mal in Gruppe III beobachtet. Nicht beurteilbar war der Effekt in einem Fall der Gruppe I und in 3 Fällen der Gruppe II. Die Wirksamkeit der Herzglykoside allein war gegenüber der zusätzlichen Gabe von Chinidin oder Flecainid signifikant unterlegen (p<0,05; CHI2-Test und Fisher-Test). Relevante Nebenwirkungen traten in je zwei Fällen der Gruppe I und III, dagegen in 8 Fällen der Gruppe II auf.
Schlußfolgerungen:

Eine Dauertherapie mit Herzglykosiden verhindert nur in Einzelfällen paroxysmales Vorhofflimmern und ist einer zusätzlichen Therapie mit Chinidin oder Flecainid unterlegen.

Bei gleicher Wirksamkeit von Chinidin und Flecainid erscheint Flecainid mit weniger Nebenwirkungen belastet.
Wegen des bisher kurzen Verlaufszeitraumes sind die mitgeteilten Ergebnisse als vorläufig zu betrachten.
