Summary
In a randomized trial, in patients with chronic atrial fibrillation, we studied the acute and preventive efficacy of a combined treatment with quinidine and verapamil, and compared the results to those obtained with amiodarone. Patients were considered for the study when atrial fibrillation was documented as being permanently present for more than 3 weeks, but less than 2 years. Acute treatment included, in group I, the administration of quinidine 1.5 g/day for 6 days and, beginning at day 3, the additional administration of 80 mg t.i.d. verapamil in fixed combination; when sinus rhythm was established. Long-term treatment was given for 1 year with 160 mg t. i. d. quinidine base and 80 mg t. i. d. verapamil. In group II, patients received 1200 mg/day amiodarone intravenously for 3 days, followed by 800 mg/day orally for an additional 11 days. When sinus rhythm was established, treatment was continued with 200 mg/day amiodarone for 3 months and then changed for a follow-up of 1 year to the above mentioned combination of quinidine and verapamil. Sinus rhythm was established in 4/9 patients with quinidine/verapamil and in 2/10 patients with amiodarone. Of six patients considered for the long-term treatment, only one patient had a recurrence of atrial fibrillation. Side effects were more pronounced with amiodarone compared to the combination of quinidine and verapamil. However, side effects did not limit therapy in any patient.
Zusammenfassung
In einer randomisierten Studie wurde die akute und präventive Wirksamkeit der Kombinationstherapie Chinidin/Verapamil mit der von Amiodaron bei Patienten mit chronischem Vorhofflimmern verglichen. In die Untersuchung wurden bisher 19 Patienten mit chronischem Vorhofflimmern, welches seit mindestens 3 Wochen und nicht länger als 2 Jahre persistierte, eingeschlossen. Die Akuttherapie umfaßte in der ersten Patientengruppe die Gabe von 1,5 g/die Chinidin über 6 Tage, ab dem 3. Tag erhielten die Patienten zusätzlich 3 x 80 mg/die Verapamil; bei erfolgter Konversion zu Sinusrhythmus wurde die Therapie mit einer fixen Kombination von 3 x 160 mg/die Chinidinbase und 3 x 80 mg/die Verapamil weitergeführt und die Patienten über 1 Jahr nachbeobachtet. In der zweiten Patientengruppe erhielten die Patienten 1200 mg/die Amiodaron intravenös über 3 Tage und anschließend 800 mg/die oral bis zum 14. Tag. Bei erfolgter Konversion zu Sinusrhythmus wurde die Therapie mit 200 mg/die Amiodaron bis zum 3. Monat und anschließend mit der o.g. Chinidin/Verapa- mil-Kombinationstherapie über insgesamt 1 Jahr fortgeführt. Eine Konversion zu Sinusrhythmus zeigte sich bei 4/9 mit Chinidin und 2/10 mit Amiodaron behandelten Patienten. Von den 6 in die Langzeittherapie übernommenen Patienten kam es lediglich bei einem Patienten unter Amiodaron zu einem Rezidiv des Vorhofflimmerns. Die Nebenwirkungen waren unter Amiodaron ausgeprägter als unter Chinidin und Verapamil, führten jedoch bei keinem Patienten zum Therapieabbruch.
