Zusammenfassung
Die mit der rheumatoiden Arthritis verbundene Entzündung wird durch verschiedene Mediatoren wie Histamin, Kinine, oxygenierte Arachidonsäuremetaboliten und lysosomale Enzyme in Gang gesetzt und unterhalten. Entzündungsfördernd wirken neben den genannten Mediatoren auch Sauerstoffradikale, die zum einen im Verlauf der bei rheumatoider Arthritis vermehrt auftretenden Arachidonsäureoxygenierung entstehen und zum anderen im phagozytierenden Leukozyten gebildet und aktiv ins umgebende Medium sezerniert werden. Diese freien Radikale fuhren im Gelenk zur Depolymerisierung der Hyaluronsäure in der Synovialflüssigkeit, wodurch diese an Viskosität und damit ihre biologischen Eigenschaften verliert. Sauerstoffradikale wirken darüber hinaus durch nichtenzymatische Lipidperoxidation und den Abbau von Kollagen entzündungsfördernd (2, 7, 15–17, 20, 21). Zur symptomatischen Behandlung der rheumatoiden Arthritis werden in großem Umfang nichtsteroidale Antiphlogistika eingesetzt. Als Wirkmechanismus werden neben der Hemmung der Prostaglandinsynthese auch die Herabsetzung der Bildung chemotaktischer Lipoxygenaseprodukte, die Stabilisierung von Lysosomenmembranen sowie das Abfangen oder die Hemmung der Bildung freier Sauerstoffradikale angenommen (1, 13, 18). Das Abfangen dieser freien, sehr reaktionsfreudigen Radikale kann jedoch auch auf anderen Wegen erfolgen. Zum einen vermindern Enzyme wie Superoxiddismutase die Konzentration an Radikalen durch Dismutation von Superoxidanion zu Wasserstoffperoxid und Sauerstoff, zum anderen Antioxidantien durch Reduktion auf nichtenzymatischem Wege (21).
